Dear Middle and High School Families:

On May 10, 2021, the U.S. Food and Drug Administration authorized the emergency use of the two-dose Pfizer Covid-19 vaccine for children 12 through 15 years of age.  With this expanded eligibility, Dinwiddie County Public Schools is partnering with the Crater Health District to host vaccination clinics here for our students.  According to current CDC guidance, fully vaccinated students will not have to quarantine when exposed to a person that has been diagnosed with Covid-19 unless the exposed student is symptomatic.

The vaccine clinics will be held as followed during school hours:

Parental/Guardian consent is required in order for a child to receive the Pfizer Covid-19 vaccine.  Attached to this letter are the vaccination consent and screening forms.  These forms may also be completed online.  The links, along with the Pfizer Covid-19 information fact sheets, can be found below.

If you wish to have your child vaccinated, complete the form in its entirety (or online) and return to your child’s school nurse no later than Monday, September 20, 2021.  Please contact the school nurse if you need assistance completing the forms.  The Crater Health District will not administer the vaccine to students with incomplete forms.

Estimadas familias de las escuelas intermedias y secundarias de Dinwiddie:

El 10 de mayo, la Administración de Drogas y Alimentos de EE. UU. Autorizó el uso de emergencia de la vacuna Pfizer Covid-19 de dos dosis para niños de 12 a 15 años de edad. Con esta elegibilidad ampliada, las Escuelas Públicas del Condado de Dinwiddie se están asociando con el Distrito de Salud de Crater para albergar clínicas de vacunación aquí para nuestros estudiantes. De acuerdo con la guía actual de los CDC, los estudiantes completamente vacunados no tendrán que ponerse en cuarentena cuando se expongan a una persona que haya sido diagnosticada con Covid-19, a menos que el estudiante expuesto tenga síntomas.

Comuníquese con la enfermera de la escuela si necesita ayuda para completar los formularios. El Distrito de Salud de Crater no administrará la vacuna a los estudiantes con formularios incompletos.
